Our mission
Palliative gr is committed to ensuring that palliative care is available and accessible in a timely, comprehensive and uncomplicated manner for all people in an advanced palliative care situation and their relatives in the canton of Graubünden. Wherever possible, this should take place at the place of choice of the person concerned. The extent and intensity of care and support can vary individually and correspond to the needs of the person concerned.
palliative care unit graubündenThe palliative care unit is part of the internal medicine department at the Cantonal Hospital of Graubünden. Patients with a terminal illness or chronic pain receive interdisciplinary and comprehensive care in the unit at the Fontana site. The stay on the palliative care ward is intended to support patients with a complex situation (pain symptoms, decision-making in a palliative situation) and enable a return to the patient's preferred environment. In addition to the inpatient ward, there is also an outpatient consultation hour in which patients with pain, nutritional problems and similar issues can be cared for.
Palliative bridge serviceThis outpatient service has been offered as a pilot project in the canton of Graubünden since January 1, 2013. The staff of the Palliative Bridge Service support and advise patients, their relatives and the people and organizations involved in care and support on complex issues relating to care and support. The specialist support can be requested by the patient, relatives, family doctor or Spitex services and nursing homes. The palliative bridging service provides additional support on a temporary basis until the carers involved are able to cope with the situation themselves.
Hospice GraubündenIn the canton of Graubünden, terminally ill and dying people can be cared for in the hospice in Maienfeld. The facility has four beds and is attached to the Senesca care center in Maienfeld.
It is a milestone in the care of terminally ill and dying people in Graubünden:The Graubünden Hospice closes a gap in the palliative care network, especially for younger seriously ill people. Patients and relatives can find competent support during a respite stay or in the last phase of life.
Voluntary support for the seriously ill and dyingVoluntary companionship for the seriously ill and dying is available to seriously ill people and supports and relieves their next of kin with the aim of caring for the patients concerned at home until they die. The motto is: "Live well until the end and die in peace." This important and demanding work is carried out exclusively by volunteers. The free care can be provided for hours or night watches.

The Bereavement Café is a service for people who are grieving the loss of a loved one. The grief café is an open and protected place to talk, listen to each other, grieve, but also to gain new impetus, courage and strength.
